MEDICALS REQUIRED - ALL MUST HAVE BEEN DONE AFTER FEBRUARY 1, 2026

Bloodwork: Hepatitis B Surface Antigen, Hepatitis C Antibody, HIV. You must have these specific tests done. We recommend using Request a Test, and ordering the Combative Trio, which includes the three required tests for $89.

Physical must be on this form, dated no earlier than February 1, 2026. If you have an IKF or other physical that was done within that six month timeframe, please foward so we can ask the commission to review.
