MEDICALS REQUIRED - ALL MUST HAVE BEEN DONE AFTER FEBRUARY 7, 2025

Bloodwork: Hepatitis B Surface Antigen, Hepatitis C Antibody, HIV. You must have these specific tests done. We recommend using Request a Test, and ordering the Combative Trio, which includes the three required tests.

Physical: This can be done by an MD, DO, Nurse Practitioner or PA. Here is the form if you need it. (Yes, it is an Oregon form, but the commission for this event uses the same form).

Eye Exam: This can be done by an opthamologist or optometrist. Here is the form if you need it.
